KAK bolt issues

Recently replaced my old BCG with a new KAK one. Dropped in with no issues but on my first trip to the range with it I noticed my magazines were having a hard time inserting with the bolt closed.

Didn't think much of it and racked the charging handle just to have rounds getting stuck, double feeding, and getting crushed. It would occasionally run for about 5 rounds before issues would persist.

Swapped the bolt out of the carrier with the bolt from my old carrier and it runs flawlessly.

Is this a headspacing issue with the new bolt? I've gone through multiple bcgs over the years and this is a first for me. Love all my other KAK products so not sure if I just got a dud this time.
