In your opinion, what was Dexter's worst Code violation?

In terms of either being a terrible thing that happened, or even just indirectly leading to terrible things that happen, what would you say is Dexter's worst violation of The Code? (it can be any violation from The Code, not just the rule about only killing murderers).
